Diabetic Supplies Coverage and Payment Rules
Home glucose monitors and supplies are covered for patients meeting the conditions below. Patients must be capable of being trained to use the device. The device is designed for home rather than clinical use.

• Patient must be a diabetic using insulin injections.
• Documentation provided by a physician of poor diabetic control, including:
• Widely-fluctuating blood sugars before meal time
• Frequent episodes of insulin reactions or evidence of frequent significant ketosis.

Insurance, Medicare Reimbursement
The Prescription Center will handle all of your paperwork to receive Medicare or health insurance reimbursement where applicable. The Prescription Center accepts assignment from Medicare and many private insurance companies. We also handle most insurance prescription cards by communicating via computer directly to each insurance company.
